Absentee Ballot Request Details

Absentee ballots are voting papers that allow individuals to vote by mail instead of in person at a polling station. They are typically used by voters who are unable to physically attend the polling place on Election Day due to reasons such as difficulty traveling, illness, or military service. Absentee ballots must be requested from the Polk County Auditor's Office. A ballot is requested via the Absentee Ballot Request Form. A printable form is linked below. The request form does not need to be witnessed or notarized. This form must be brought to your County Auditor
